Centerwell rolls out a new model of care for seniors

Senior care, a new model in Greenville



CenterWell Senior Primary Care operates under a value-based care model. This approach represents a new concept in care compared to traditional fee-for-service models. 


In essence, fee-for-service providers get reimbursed based on the quantity of services they provide, not on the quality or outcome of care. The new model does much more for patients at no extra cost to the insurance carrier or to the patient.

The target market is patients over 64.  These seniors have special needs, especially as their conditions multiply and become chronic.


In fact, the sicker you are, the more patients experience a fuller impact of care that matches their growing needs.


This is a specialist and a new model.  That’s why a limited number of Medicare Advantage Plans work with this concept.


Who are the staff doctors?


There are several doctors at CenterWell.  All are primary care and board certified doctors. As needed, they refer patients to various specialists.

American Board of Family Medicine 1998.


Dr. Joshua Paul joined CenterWell as a board-certified family medicine specialist. Dr. Paul completed his family medicine residency at Saint Joseph's Medical Center in Yonkers, New York. He earned his medical degree With Honors from the Medical University of South Carolina in Charleston, South Carolina, in 1994. Dr. Paul developed an interest in healthcare at an early age when he traveled extensively with his father to deliver medical help to the poor in Haiti. Now a multi-decade healthcare industry veteran, Dr. Paul defines a successful doctor-patient relationship as one of mutual trust, where the patient feels part of the decision about their healthcare. Dr. Paul is fluent in English, French, and Spanish.


 



  • Patients must be older than 64 and have Humana, CIGNA, Wellcare, or Aetna plans to participate.


The result is more personalized care with better outcomes. Care escalates for critical-care patients with COPD, diabetes, and congestive heart failure.  


There are several nurses, all trained to specially help seniors.  Today we focus on one nurse.
